Medicare Clarity Now is operated by Chauncey Green, a sole proprietor based in Concord, California. This policy explains how information is collected and used through medicareclaritynow.com.

Information you provide

When you request personal assistance, we may collect your name, email address, telephone number, ZIP code, county, preferred contact method, current coverage, provider or medical-group name, important facilities, and the message you choose to submit.

When you request the Sutter Medicare checklist, we collect the contact information needed to deliver it and administer the request. Permission for personal Medicare follow-up is requested separately and is optional.

Do not submit a Medicare number, Social Security number, full date of birth, financial information, medical records, diagnoses, prescription names, or passwords through this website.

How checklist requests are handled

A request for the educational checklist authorizes delivery of that checklist only. Checklist-only visitors are not automatically added to Medicare marketing campaigns and are not called or sent additional Medicare marketing unless they separately request personal follow-up.

Information collected automatically

Hosting and security services may process IP addresses, device and browser information, request logs, timestamps, referring pages, and security signals. If optional analytics are enabled, the site may collect general usage information such as pages viewed, approximate location, device type, and campaign parameters.
